Parent's Release and Indemnity Agreement

We (I) request that you accept the application for enrollment of our (my) son/daughter named above in the Minnesota Baseball Instructional School during the dates set forth in this application, and in consideration of your acceptance of this application:

We (I) hereby release the Board of Regents of the University of Minnesota, Minnesota Baseball Instructional School and all of their employees from all claims on account of any injuries which may be sustained by our (my) son/daughter while attending the Minnesota Baseball Instructional School;

We (I) agree to indemnify Board of Regents of the University of Minnesota, Minnesota Baseball Instructional School and all of their employees for any claim which may hereafter be presented by our (my) son/daughter as a result of any such injuries;

If medical attention is required for injury or illness while in camp, we (I) give our (my) permission for such medical care;

We (I) grant permission for the Minnesota Baseball Instructional School to use photographs of our (my) son/daughter for publicity, advertising or other commercial purposes.
